Maintenance and review of patient medication profiles; reviewing and verifying physician medication orders and patient medical record; and checking of IV products. Interacting with healthcare professionals and providers to help promote safe medication use, prevent inappropriate or misuse of medications; provide clinical pharmacy services (e.g., pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, clinical protocols per pharmacy and pharmacist interventions). Schedule: Schedule: Monday-Friday, 8:00 am - 5:00pm (hours may vary based on departmental needs). You are required to have the ability to travel to multiple work sites within UCLA Health. Hourly Salary Range: $85.87 - $100.58 (Union represented, step based salary depending on years of experience) Bargaining Unit: HX-UPTE Qualifications We are seeking an exceptionally-skilled and compassionate pharmacist with the following qualifications: + Doctor of Pharmacy Degree + California Pharmacy license + ASHP-accredited PGY2 oncology residency or at least a minimum of 1 year of pharmacy practice experience in oncology + Recent clinical oncology experience (within 5 years) + Board Certification in Oncology (BCOP) preferred + Previous infusion pharmacy experience preferred + EPIC/Beacon experience preferred + Exceptional communication, interpersonal, organizational and follow-through skills + Knowledge of and ability to perform age specific and disease specific pharmacokinetic calculations + Required to have the ability to travel to multiple work sites within UCLA Health UCLA Health welcomes all individuals, without regard to race, sex, sexual orientation, gender identity, religion, national origin or disabilities, and we proudly look to each person's unique achievements and experiences to further set us apart.
